Whoever rotated the tires didn't reset or "match" the sensors back up. This matching process has to be done every time you change a tires location on the vehicle.(if you want them to read the correct location)

Not sure what you've been doing the other times you rotated tires. Me. I rotate my own, then reset the TPMS per the owners manual.
