You could try googling the model number for a manual. There may be a reset button or something simple.

Links to their 'owners manuals' looks like the tiny pamphlet you got is all there is. But looking at the wiring instructions, it seems pretty simple, just a few wires. Could be a bad connection or defective equipment.

After looking at those books I am wondering maybe you have LED backup lights?

Usually the camera draws power from the backup light, so it is only on when in reverse. A LED might do odd things in that sort of circuit.

Or, the installer is just a dope.

I can't see a way to screw it up.
