Blown Speaker
Wow...that's a bummer...but from what I hear, these are not really great speakers.
From what I remember seeing on the forum, there is a screw inside the door pull that holds it to the door frame and inside the handle area that opens the doors. The rest I believe are metal clips that hold the rest of the door panel to the door itself. Captain Howdy had a thread about how to pull these, but it's been among the missing. Maybe he'll see it and post some photos for you if this doesn't help.

I've had mine cranked all the way and never had that happen...maybe it's the music. When mine is cranked that loud it's usually Three Doors Down or Lenny Kravitz. 

I did my speakers a while ago, but as I remember, there are also a couple of push-pins holding the panal on. They are on the side facing the back of the car. You have to push the center pin in a little, then pry the whole thing out and reset it.

After you remove the screws with a torx bit, you can pull the door panel off.
since you are going to replce one speaker you might as well replace all four. The front and rears are the same size. If you have the LS you will have to get some schosce (spelling?) speaker brackets to mount the new speakers.
