I do everything I can to plan ahead, but still you can count on someone refusing to let you in.
Usually I signal, expecting them to bolt, and then take the hole they make for me.
I had someone recently refuse to let me over, when the freeway was narrowing, and I had nowhere to go but the border. Speed up/Slow down, he just kept pace. And then when I was off the road, he lane changed 2 to the left. Never once met my eye, or gave me an indication that he knew I was there. Blasted Beemer!

Eh... That's why I drive defensively 100% of the time on the highway- people either aren't paying attention or they behave as has been mentioned here. I always use my blinker, but only as I start my merge into the lane I need...
