Hey everyone, thank you for the replies and the various links and information.
As RJ_RS_SS_350 already mentioned when you use the link at CarID it has you select the engine and model and then displays the correct item, in my case the item no. is listed ACD 15-81765 with the alternate number being 15940324 which matches what I see in the other links, see image of product details listed on my invoice below.
I also thought it was odd that the ACDelco branded part was cheaper than some others on the CarID site, the same unit was nearly $300 in a couple of other places but very similar to what's listed on rockauto.com (rockauto actually a few bucks cheaper).
I'm leaning towards the problem being the Fan or the Module even though I purchased them as a single unit new and shouldn't be an issue. First thing is to verify those voltages tomorrow and second is to disconnect the module from the Fan terminals and apply 12 Volts to the Fan terminals to see if it spins freely.
I'll update this thread with the results, thanks again for your help and info on this.

With Rock Auto you could have used the 5% discount also.
They are a vendor on this site.
I know this info is after the fact of you buying the part.

Hey everyone, I went ahead and took some voltage measurements at the connector that plugs into the Fan Control Module, below are the results.

Engine off:
Pin 1 to 2 = 12.17 V
Pin 4 to 2 = 3.48 V
Engine Running:
Pin 1 to 2 = 14.13 V
Pin 4 to 2 = 3.52 V * turned A/C on and voltage at Pin 4 dropped to 1.06 V with the coolant temp at 162 F
When the coolant temp hit about 180 F the voltage dropped to 0.25 V and stayed there until the temp hit 220 F, at which point I turned on the heat full blast and shut down the engine shortly after.

Not sure what to make of the voltage readings as I never saw 12 V at Pin 4, hoping it's not an issue with the ECU/ECM unit not providing the correct voltage to Pin 4.
I'm thinking it's a bad FCM even though it's new, basically dead on arrival, and I've contacted CarID to RMA the unit and get a replacement. They asked for some info, including a couple of images of the unit in question, which I thought was odd but I'm now waiting for the RMA.
Have to send complete assembly back in the original packaging and pay the $9.50 shipping then they will ship the replacement once they receive the one I'm returning, this part is frustrating because it means I'll be waiting at least 4 or 5 Days for the replacement.
I'll keep you all posted on the results and again thanks for the info and ideas.

Well I finally received the replacement Fan Assembly from CarID and that resolved my issue and the HHR is no longer running hot, it actually appears to be running cooler than normal as it's usually around 185 to 190 over the last 3 drives since replacing it Thursday evening including one time yesterday afternoon when I was in stop and go traffic for about 15 minutes because of road work.

As Donbrew mentioned in a post above, "problem is with the module or the fan or the wire between them", it appears that indeed it was and I'm assuming it was the Fan Control Module since the Fan did spin freely.
I noted with this new Fan Assembly that the Fan comes on at 190 F with the A/C off which appears to be early but appears to be consistent and it also throttles up as required and you can notice it by the Fan noise that it's speed increases.

Since it's been hot out and I have the A/C always on the Fan is always on as well and the temp as been very steady since Thursday which is a big relief and all seems well.
