Oil leaking into Coolant

I got the old engine out and I still can't figure out what really caused the oil to leak into the coolant. The oil was clean when I drained it with no signs of coolant. I pulled the head off to see this, but I can't tell if it was leaking at the head gasket. It doesn't look good for only 5000 miles on the gasket

Perhaps we can see from the underside of the cylinder head?

My guess is bad valve guides. There is a lot of oil in the combustion chambers.

That, and possibly oil being sucked through the hose from the valve cover to the elbow at the throttle body, but that doesn’t explain coolant and oil. What about leaking water pump?

I forgot to take a picture of the head but here it is. The oil started dripping down the head yesterday so that cyl 4 oil is just from the oil dripping. The cylinder head is a reman one that was installed at the same time as the head gasket

I replaced the water pump with one from ZZP last year and it didn't fix the issue

Looks like a piece is broken out, far right between cylinder 1 and the timing chain area.

Or is that just residual gasket material?

The intake valves on cyl #3 look odd. That’s some strange deposits. Especially the intake valve on the left. It definitely looks like all cylinders are getting oil burned in them. Doesn’t look like carbon.
Is it possible to mix up cylinder head gaskets ? I mean can you put a 2.4 gasket on a 2.2 by mistake ? The bore is only slightly different between the two. 86mm vs 88 mm.

I can see how it looks like the head is broken. I checked and it's just the gasket material. That's a good idea that maybe a 2.4 head gasket is on there. I measured the ID of the head gasket and it's just under 87mm, so I think it matches up with the 2.2 bore.
