I recommend contacting your local Chevrolet dealership about the reimbursement process. According to the information I have available to me, a receipt/invoice needs to be provided in addition to the forms that need to be filled out. The dealership then process these forms.

^ yup you need the invoice of the work that was done and the letter you got in the mail. I got my check in the mail couple weeks ago.

I got the letter about a month ago and set up an appointment at the dealer to have the work done...easy and no out of pocket. I had not had any problems but why take chances right? I have a 2010 hhr ss.

I had mine done a few months back, I had to pay over $300 out of my own pocket to fix a known problem!!! I have an '08 SS, and at the time only had 30,000 miles on it, but warranty, apparently not, I think that's BS! Love the car anyways

Guess thats where the 3 year or 36K comes into play.. I have an 08 also..

The tech rep at Martin Chevrolet in Torrance, CA took one look at the letter and had the cylinder swapped out in a hour no questions asked. Nice follow through GM.
